Required Athletic Paperwork – FHSAA Requirements
Before participating in any athletic programs next year, all student-athletes must first complete their Florida High School Athletic Association (FHSAA) requirements. This needs to be completed each year. There are two (or three) forms that must be completed before any student-athlete can practice or try out for a team. Below is a brief description of each form.

FHSAA Form EL1 – Electrocardiogram (ECG) Screening Form: This form must be completed for all incoming 9th graders and new student-athletes in grades 10-12.

FHSAA Form EL2 – Preparticipation Physical Evaluation: This form requires a doctor visit for a brief exam to ensure that every student-athlete is healthy and ready for a season of physical activity. Please complete page one on your own. The doctor will complete page 2 (and page 3, if necessary).

FHSAA Form EL3 – Consent and Release from Liability Certificate: This four-page form informs you of the potential dangers associated with participating in a sport. All four pages must be read and digitally signed by both the student-athlete and a parent or parents through the Athletic Clearance Home Campus platform.
Need a Physical? On-site physicals will be coming in August!
The Athletic Department will once again offer on-campus physicals to help student-athletes complete their necessary paperwork. Due to schedule constraints, the physicals cannot be offered until August. This regards the EL2 paperwork. We are also looking to include the ability to complete the necessary EL1 (ECG Screening) form for incoming freshmen on-site. More details will be shared as the date approaches.
